Alvarez has been at St. Clare’s for more than twenty-five years, accompanying Harmon-area families through the weeks between a diagnosis and a death, roughly a hundred and forty-five of them by the count she keeps, because families are her metric where the institution prefers cases. She arrives when the surgery is over, or when the surgery was never going to happen. She sits in rooms with no monitors and tells people the truth at the pace they can carry it. The surgeons know her the way Neil Mercer does, with a relief that is its own tell: glad the conversation belongs to someone else.
She is the series’ quiet counterweight to its measurable medicine. Neil Mercer builds a career on the resection, the margin, the millimeter (The Steady Hand); Alvarez builds one on the part that cannot be measured. Cat Brennan does the catching; Alvarez does the long accompanying (The Tired Mother), and it is Alvarez who comes on rounds to manage Eleanor Morrow’s last eleven months in The Distant Man, and Alvarez who sat the Torres family in a windowless room and was right about every part of the timeline in The Lawyer Daughter. In The Merciful Nurse she is the attending Luisa Dominguez asks the hypotheticals that are not hypotheticals.
Her reach widens with the series. She served fifteen years on the Pine Ridge medical advisory board, and by the later books she is the medical director of the renovated Hartwick Memorial Hospice, the 1948 sanitarium Anne Loring’s firm restores. She is, as one character files her, “the medical witness in the network,” the recurring physician three books’ worth of dying pass through, the doctor who treats the part of medicine that is not the saving but the not-leaving.
